Consent Form
Before your child attends the TAC, we need a few details from you so we can provide the best service for them. You can click on the document icon to download the consent form. (All 8 pages need to be filled in.)
Or you can complete the online form instead. By sending/signing the consent form, you are confirming that you have read, understood and agree to the terms, conditions and policies as stated in the Parent Information Pack below.
Our Parent Information Pack, which will contain our terms, conditions and policies, is currently in preparation.
It will be substantially the same as the ZAC policies, which are available on the ZAC website.
